Foam or gel — which Spectrum should I choose?

The Foam version has no gel or fluid insert, so there is nothing to check or redistribute, and it carries a higher weight rating (294kg). The Gel version has a lightweight fluid insert in the pelvic well. Your OT can advise which suits you.

Is the cover waterproof?

The CoolCore cover manages moisture, and there is a urethane liner underneath it for incontinence.
